    This notice is published pursuant to 22 TAC §279.16(d)(3), which requires optometrists providing telehealth services to disclose complaint procedures on their practice website or within informed consent materials prior to rendering services.

    This notice is published and maintained by Riz Eye Care in compliance with the telehealth disclosure requirements of Texas Administrative Code §279.16 and Texas Occupations Code Chapter 111. All telehealth services rendered at Riz Eye Care locations are provided under the personal supervision and clinical authorization of Rizwan Jaffer, O.D., Therapeutic Optometrist and Optometric Glaucoma Specialist, licensed by the Texas Optometry Board. This notice is reviewed and updated no less than annually and immediately upon any material change to applicable Texas Optometry Board rules or Board guidance affecting telehealth services. Effective: April 2026. Next Scheduled Review: April 2027.
