Ozempic Vision Loss Screening in Houston

Medically reviewed by Dr. Rizwan Jaffer, O.D. — July 2026

GLP-1 medications like Ozempic, Wegovy, and Mounjaro have been linked to NAION — a sudden, permanent form of vision loss. A baseline screening every 6 months creates a record of your eye health and catches warning signs early.

    What is NAION?

NAION — non-arteritic anterior ischemic optic neuropathy, sometimes called an "eye stroke" — happens when blood flow to the optic nerve is reduced. The result is sudden, painless, and usually permanent vision loss in one eye. Most people notice it the moment they wake up.

    Warning signs to know:

    • •Sudden dim or dark vision in one eye
    • •A shadow or curtain covering part of your visual field
    • •Blind spots that appear overnight
    • •Loss of peripheral vision on one side

Any of these symptoms should be evaluated the same day — not at your next scheduled visit.

    The Science

    2024 · JAMA Ophthalmology

A peer-reviewed cohort study found an increased risk of NAION in patients prescribed semaglutide compared to matched patients on other therapies.

    2025 · EU Regulators

The European Medicines Agency added NAION to the official semaglutide product label as a very rare side effect.

    U.S. Litigation

U.S. lawsuits alleging GLP-1–related vision loss have been consolidated into federal multidistrict litigation.

We describe this association as "linked to" rather than "caused by" — that language reflects the current medical and legal record.

    Automated visual field testing

Detects subtle peripheral or altitudinal field defects characteristic of optic nerve injury.

OCT of the optic nerve (RNFL)

Objective measurement of retinal nerve fiber layer thickness — the earliest structural sign of nerve damage.

    Fundus photography

High-resolution color photos of your optic nerve and retina to compare against future visits.

    Pupil testing

Screens for a relative afferent pupillary defect (RAPD), a classic NAION finding.

    Dilated optic nerve evaluation

Personally performed by Dr. Rizwan Jaffer, O.D., not delegated to a technician.

    Written baseline report

You leave with a documented record of your eye health — medically and, if it ever matters, legally.

Why it matters: a documented baseline is the only way to prove when your vision was normal.

If vision changes ever appear, you already have the objective record — imaging, fields, and clinical findings — that no test can recreate after the fact.

Wegovy, Mounjaro, Zepbound and Rybelsus Vision Risks

Ozempic (semaglutide) is the drug most people have heard of, but the concern extends across the full GLP-1 receptor agonist class. Wegovy is the same molecule as Ozempic — semaglutide — prescribed at higher doses for weight loss, and it has been linked to the same NAION signal in the peer-reviewed literature. Rybelsus is oral semaglutide; the route of administration differs, but the active drug is identical. Mounjaro and Zepbound are tirzepatide, a dual GIP/GLP-1 agonist from a different manufacturer; the ischemic optic neuropathy signal is less mature for tirzepatide than for semaglutide, but the mechanistic concern — reduced perfusion to a small, crowded optic nerve — is shared enough that we screen tirzepatide patients under the same protocol. If you have taken any of these drugs, even briefly, a documented baseline eye exam creates the record that no imaging can reconstruct later.

Vision Loss After Ozempic — Warning Signs to Watch For While Taking GLP-1 Medication

NAION does not build gradually. It arrives, most often, in the minutes after you open your eyes in the morning. Four patterns to know: sudden dim or greyed-out vision in one eye that does not clear when you blink; a shadow or dark curtain covering the upper or lower half of your visual field; a fresh blind spot noticed on waking, sometimes described as a "hole" in what you can see; and loss of peripheral vision on one side, especially the nasal or lower field. There is no pain. There is no red eye. Because NAION is painless and one-sided, patients frequently dismiss it for a day or two — and that delay is exactly when the window for evaluation closes. If any of these appear while you are taking Ozempic, Wegovy, Mounjaro, Zepbound, or Rybelsus, be seen the same day.

Why a Baseline Eye Exam Matters Before Problems Start

NAION has no known reversal. Once the optic nerve loses perfusion, the tissue that dies does not return, and vision that is gone at diagnosis is generally gone for good. That reality shifts the value of an eye exam from treatment to documentation. A baseline screening done today captures objective measurements — retinal nerve fiber layer thickness on OCT, Humphrey visual field sensitivity across the entire retina, high-resolution optic disc photography, and pupil reactivity — while your vision is still normal. If vision changes appear months later, the comparison is direct: what was measured then versus what is measured now. Without a baseline, that comparison does not exist, and every conversation with a neuro-ophthalmologist or attorney becomes a reconstruction. With one, the timeline speaks for itself — medically and, if the situation ever requires it, legally.

Diabetic Eye Exams vs. NAION Screening — What's the Difference

Patients on GLP-1 medications for type 2 diabetes often assume their annual diabetic eye exam already covers this concern. It does not. A diabetic retinopathy exam is focused on the retina — the light-sensing tissue at the back of the eye — looking for microaneurysms, hemorrhages, and macular edema caused by poorly controlled blood sugar. NAION is a disease of the optic nerve, a different structure entirely: it is the cable that carries visual signals from the eye to the brain. The tests are different too. Diabetic exams rely on dilated fundus photography and OCT of the macula. NAION screening adds OCT of the retinal nerve fiber layer around the optic disc, automated Humphrey perimetry to map subtle field defects, and a swinging-flashlight pupil test to detect an afferent defect. Both matter. Neither substitutes for the other.
